우리나라의 고위험임신과 신생아의 국가적인 관리를 위해 전국에 고위험산모신생아통합치료센터가 2015년 개소한 이래로 벌써 6년이 흘렀고 전국에 21개 센터가 개소되었다. 각 센터에서 일반적으로 치료를 받는 고위험산모와 고위험신생아의 환자분포, 질환의 중증여부에 따른 치료결과, 합병증 발생 빈도 등 객관적인 통계자료는 아직 없는 상황이며 이러한 자료를 얻기 위해서는 표준화된 진료 지침이 우선 제시되어야 할 것으로 생각된다. 본 역서는 우리나라보다 약 10년 먼저 전국단위로 고위험산모신생아통합치료센터를 설치 운영하였던 일본의 각 기관 대표자들이 모여서 ‘전국 주산기의료 연락협의회’에서 만든 ‘MFICU 매뉴얼’ 3판을 기준으로 하였다. MFICU에서 만나게 되는 비정상임신, 내 외과적 합병증임신, 태아기형, 비정상분만, 산욕기 이상 등 다양한 산과적 진단 및 치료 와 신생아 처치에 대한 기본적인 개념 및 중요한 점검항목에 대하여 기술하고 있으며, 이를 통해서 전국적인 진료의 표준화 및 그 치료결과를 비교할 수 있는 기준이 되는 참고자료라 생각한다. 물론 한국과 일본이 가장 지리적으로 가까운 이웃나라로 지형, 기후, 문화, 생활습관이 유사하지만 한국과 일본의 의료제도, 진료지침 및 의료보험 적용 등이 다르기 때문에 이 책 내용을 그대로 인용하는 것에는 한계점이 있으며, 국내 의료 현실과 다를 수 있다. 하지만 고위험산모의 특징이나 질병발생빈도 치료성적 등은 우리가 참고할 수 있는 부분이 있으며 어떤 질병군이 고위험산모신생아통합치료센터에서 다루어지고 있는지는 우리나라에서도 돌아볼 필요가 있는 점이라 생각한다. 책의 내용을 번역하는 과정에서 나오는 의학용어는 가능한 국내에서 사용하는 단어로 바꾸어 번역하였으나, 일본에서만 허용되는 약물 및 대체가 어려운 용어에 대해서는 원문에 충실하게 옮기었다. 이 책이 나오는 데 일차번역과 교정을 보아준 길병원 고위험산모신생아 통합치료센터 김시라 코디네이터의 노고에 고마움을 전하며, 내용을 교정하고 정리해준 길병원 산부인과 임찬미, 정다회, 전해린 선생님과 책의 인쇄 교정 출판을 성심껏 해주신 군자출판사에 감사함 마음을 드린다. 이 번역서가 각 고위험산모신생아통합치료센터에서 진료하는 과정에서 가까이 놓고 쉽게 찾아보면서 늘 참고할 수 있는 서적이 되길 바라는 마음이다. 2021년 가을 가천대길병원 고위험산모신생아통합치료센터 김석영
