“The Ventilator Book” 은 학생, 전공의, 간호사, 호흡치료사를 위한 안내서이다. 기계환기에 대해 빠르게 찾아보고 쉽게 읽을 수 있는 개요를 쓰는 것이 목표였다. 독자들의 반응을 보면 “The Ventilator Book” 을 쓰면서 계획했던 목표는 잘 달성되었다고 확신한다. “The Advanced Ventilator Book” 은 전편인 “The Ventilator Book”을 참고하여 형식과 구조는 동일하게 유지하면서 독자들을 한 단계 끌어올리는 것을 목표로 썼다. 이 책은 중환자치료에 어느정도 경험은 있지만 중증호흡부전환자의 치료방법에 대한 지침을 원하는 임상의사들을 위해서 쓴 책이다. 나는 이 책의 독자들이 기계환기의 기본적인 원리, 중증질환 또는 손상의 병태생리를 잘 이해하고 있다는 가정하에 이 책을 썼다. 처음 두 장은 기본으로 돌아가서 산소공급에 대한 개요와 허용적 고탄산혈증(permissivehypercapnia)의 개념을 다뤘다. 그 다음으로 호기말양압의 적정(titration),중증기관지연축환자의 치료, 복와위자세(prone positioning)와 치료목적의 신경근차단제, 흡입산화질소 및 흡입프로스타사이클린, 정맥-정맥 체외생명유지장치(veno-venous extracorporeal life support), 그리고 이 모든 것을 통합한 치료전략의 순서대로 기술하였다. The Ventilator Book의 한 가지 특징은 실용성에 대한 강조였다. 많은 교과서와 논문들이 특정한 환기모드나 특정치료법의 근거를 기술하고 있지 만, 독자들에게 실제로 어떻게 해야 하는지를 알려주는 책은 비교적 드물었다. The Ventilator Book 과 마찬가지로 The Advanced Ventilator Book도 임상의사들에게 여러 원리들을 어떻게 진료에 적용하는지에 대해 도움이 되는 단계별 지침을 제공한다. The Ventilator Book에서 강조했던 것과 마찬가지로 The Advanced Ventilator Book 에서도 완치(cure)보다는 보조(support)와 폐보호(lung protection)에 중심을 두고 설명을 계속해나간다. 따라서 약속된 마법의 총알은 당연히 없다. 왜냐하면 존재하지 않기 때문이다. 중증호흡부전환자에서 시행하는 기계환기에 의해 손상이 동반될 가능성이 아주 높으므로 이 책에서 는 각 장의 주제와 더불어 예방 가능한 손상을 피하는 것에 대해도 강조해서 설명했다. 중환자의학의 대부분은 태생적으로 보조적(supportive)인 성격을 갖고 있으며 급성호흡부전의 치료도 예외는 아니다.
