나는 지난 40여 년 동안 암 환자를 수술하고 암에 대한 연구를 하며 지냈는데, 나에게 가장 큰 의문은 ‘왜 암세포는 자신도 죽게 되는데 자신의 숙주인 환자를 죽도록 만들까?’ 하는 것이었다. 물론 진화론적으로는 다세포 생물이 하나의 세포나 장기가 망가지면 전체를 죽이는 하나의 방법으로써 암을 활용한다는 진화론적인 이론이 있기는 하지만 아직도 잘 이해하기 힘든 부분이다.
후배인 외과의사로부터 섬진강 강변을 거닐면서 쓴 편지를 읽는다. 내가 직장암으로 수술하고 재발하여 방사선요법과 천추절제, 그리고 회장뇨루 등의 수술을 하고 결국은 재발하여 돌아가신 대학 선배 의사의 치료에 대한 자신의 소회와 다른 방법을 찾기 위해 독일, 인도 등에서 통합의학적인 암치료에 대해서 공부한 배경과 그리고 자신이 암에 대해서 갖게 된 생각들을 정리한 편지이다.
히포크라테스는 인간에게 생긴 질병을 치료하기보다 병에 걸린 인간을 치료하라고 하였다. 그리고 헬렌타우시그는 ‘전자 현미경에서 고개를 들어 환자의 간청하는 눈빛을 바라보라’하였다. 내가 근무하는 병원에는 ‘先愛治患’이라는 액자가 있다. 먼저 환자를 사랑하고 질병을 치료하라는 말이다.
근대의학은 기계론적인 세계관에 입각하여 인간의 생리적 법칙을 기계의 작동 원리로 치환시켰다. 인간의 병리적 상태는 곧잘 비정상적인 기계의 고장이나 오작동으로 묘사되었으며, 나아가 정상적인 구조와 기능을 되찾기 위해 수리가 필요한 대상으로 규정한다. 환자의 생활, 병의 배경, 영혼이 부서지는 듯한 고통, 가족들의 고뇌, 회복에 대한 희망, 이 모든 것이 혈액의 나트륨 수치에 대한 지식만큼 치료의 일부이다.
내가 그동안 시행하고 암 환자들에게 권유했던 수술, 항암화학요법, 방사선요법 등의 표준치료와 최근에 새로이 암의 요법으로 각광을 받는 면역요법에서 암의 다양한 입양세포치료 방법과 면역억제 세포에 대한 표적치료, 미국의 카터 전대통령의 악성흑색종의 뇌전이 치료에 사용된 면역관문억제제 치료에 대한 관심과 함께 암의 미세환경과 이에 대한 다양한 암의 치료 방법에 대해서 공부하고 그 지식을 나누고자 하였다. 30년전 뉴욕의 코넬대에서 수술장에서 전이된 대장암의 조직을 배양액에 담아 실험실로 보내서 면역세포치료를 시행하던 DeCosse교수가 생각난다. 특히 열심히 치료하고 노력하였음에도 점차 전이되고 진행되어 죽음에 이르는 환자들을 보면서 그들에게 무엇이 필요하고 무엇을 해 줄 수 있을지를 고민하였다. 그리고 통합의학적인 방법으로 광범위하게 시행하고 있는 많은 치료 방법들과 약제들 그리고 한동안 암 환자들에게 유행한 구충제 등 다양한 치료 방법들이 과연 무엇인가? 그리고 어떤 기전에 의해서 이들이 암치료에 사용되었고, 왜 환자들에게 어필하고 사용하는지도 의문이었다. 또 암의 교과서에서 다루어지고 있는 미국의 엠디 엔더슨 암센터에서 시행하고 있는 통합적인 암치료는 무엇인가?하는 의문들이 나에게 다양한 연구들에 대한 문헌을 찾아 공부하고 정리하는 동기가 되었다.
암은 우리가 임상적으로 발견 가능한 1gm의 종괴가 10억 개의 암세포로 이루어져 있다. 암의 미세환경을 보면 혹시나 이러한 수많은 세포들의 집단 지성이 이들을 공격하는 숙주와 의사들의 노력에 반해서 생존에 관여하고 있지는 않는지 우문을 해본다. 암은 성장함에 따라서 미세환경이 변하고, 암세포는 정상세포와 그 대사작용이 서로 다르며, 공생하고 있는 미생물, 암과 숙주인 환자의 면역환경과 면역세포가 변하게 된다. 그리고 이러한 변화와 함께 환자가 섭취하는 영양과 음식물, 환자의 심적인 상태와 환자의 정신적인 의지, 환자의 운동 등이 암의 발생, 성장과 전이 그리고 치료에 관여한다. 그래서 나는 암은 하나의 치료 방법이 나 한 가지 약 혹은 방법으로 치료될 수는 없다는 생각을 갖고 있다. 미래에는 이러한 세포와 세포 주위의 환경들에 대한 자세한 작용기전이나 이러한 지식을 이용한 치료에 대한 임상 결과가 밝혀지리라 생각되지만, 현재로서는 여러 가지 방법을 잘 알아서 이들에서 가장 좋은 조합을 찾아 병용하여 치료하는 것이 가장 좋은 방법이라 생각된다. 물론 이들 방법에 대한 객관적인 임상연구가 필요하고 그 결과에 의한 근거가 중심에 있어야 되겠다.
이 책은 정통적인 암치료법에서 표준요법으로 시행되고 있는 수술, 방사선요법이나 항암화학요법에 대해서는 생략하거나 간략하게 그 원칙만 기술하였으며, 각 장기에서 발생하는 암에 대해서도 잘 알려져 있고 또한 너무 방대하여 언급하지 않았다. 그리고 암의 면역요법이나 대사, 식이, 온열요법, 운동요법, 심신요법 등의 다른 치료에 대해서도 이 책의 부족한 점이 많아서 다른 견해와 방법이 있을 수 있다. 그러나 암의 표준치료를 시행하고 있는 암 전문가와 암에 관심이 있는 많은 사람들, 그리고 통합의학적 방법으로 암 환자들을 치료하고 있는 의사들에게 환자를 이해하고 치료하는데 도움이 되고 참고가 되었으면 한다. 그리고 암 환자들이나 그의 가족들에게 참고가 되었으면 하는 바램을 갖고 있다.
21세기에는 암이 정복되리라고 많은 과학자들은 생각하지만 암의 완치는 아직 갈 길이 멀다. 내가 2년 전 고창 석정웰파크병원에 근무하기 시작하면서 집사람과 밤에 산책을 나갔는데, 높은 곳에 밝은 빛이 보여서 나는 당연히 별인 줄 알았고 별이라고 우겼다. 나중에 방장산의 높은 곳에 미소사의 한 스님이 암자에 기거하고 거기에 켜놓은 전등불이라는 것을 알았지만 말이다. 여기 기술한 내용이나 치료 방법이 암치료의 극히 일부분이고, 또한 아직 과학적으로 증명되지 않은 치료 방법도 있다. 그리고 내가 치료에 경험하지 않은 방법도 참고로 기술하였으며 과학적인 논거를 찾기 위해서 노력하였다. 그러나 알고 선택하는 것과 전혀 모르면서 어떤 치료 방법을 선택하는 것은 다르다. 여기 기술한 기전이나 원리, 이러한 방법을 이용한 임상결과들이 암 환자 치료에 도움을 주고 여러분의 고민에 참고가 되기를 바란다. 그리고 후에 훌륭한 후학이 암의 치료 방법들을 종합해서 암을 완치할 수 있는 전략을 만드는데 조그마한 도움이라도 되었으면 한다.
